Payment & Insurance
All payments for services are made at the time of your appointments. Dr. Zuckerman does not participate with insurance but can provide you with a statement of services containing the necessary billing codes. Should you wish to submit services through insurance it would be your responsibility to send the required information to your insurance company directly.
For all new patients a $50 deposit will be required to hold your evaluation appointment via credit card. The deposit will be applied to the evaluation cost at the time of your appointment. Should you cancel less than 48 hours before your scheduled evaluation, the office will retain the deposit as a late cancellation charge.
Dr. Zuckerman accepts all major credit cards, check and cash.
Cancellation Policies
Cancellations made less than 24 hours before the start of your therapy appointment will result in a charge for the full session amount. New patient evaluations cancelled 48 hours before the start of your appointment will be automatically charged a $50 late cancellation fee.
Form Requests
There are times patients may request forms to be filled out on their behalf. A small fee will be implemented based on time required to complete the request.
